Apparel Order Form ... revised

I have reposted the apparel order form so that the entire sheet will now fit into your print screen (my apologies to those who have already printed).

Click on the form to enlarge and then print off a copy (can be black/white). Return competed forms (with payment) to me by 8:00 am this Friday.

Reminder to all new players - You need to purchase a forest green sleeve to go under your uniform. You have the option to purchase one with this order.
